Insurance

We are in-network providers with Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, Coventry Wellpath, Medcost, and Medicare.  Although we are in-network, this is not a guarantee of benefits. Each policy varies.  Our team will make every effort to verify your insurance coverage as courtesy to our patients and will inform you of your covered benefits that your insurer reports to us prior to your initial consult.  It is each patient's responsibility to update our office with any change in health insurance so we can verify your new benefits. 

Blue Cross and Blue Shield 

Many of the Blue Cross and Blue Shield policies cover Medical Nutrition Therapy at 100% (no co-pay, no coinsurance, no deductible) as a part of your preventative care.   

Blue Cross and Blue Shield State Health Plan

Blue Cross and Blue Shield State Health Plan (ex. YPYW)

 

Enhanced 80/20 plan and Standard 70/30 plan…cover UNLIMITED visits to a Registered Dietitian with no out of pocket expenses. ($0 copay, $0 coinsurance, and $0 deductible)

Blue Cross and Blue Shield Federal

Blue Cross and Blue Shield Federal (ex. R) covers 6 visits each year for Medical Nutrition Therapy at 100% (no co-pay, no coinsurance, no deductible) and unlimited based on medical necessity, as a part of your preventative care. 

Cigna

Many Cigna policies cover Medical Nutrition Therapy at 100% (no co-pay, no coinsurance, no deductible) as a part of your preventative care.   

Aetna

Most Aetna policies cover 10 visits per calender year for Medical Nutrition Therapy at 100% (no co-pay, no coinsurance, no deductible) as a part of your preventative care.    

UnitedHealthcare

Many of the United Healthcare policies cover Medical Nutrition Therapy at 100% (no co-pay, no coinsurance, no deductible) as a part of your preventative care.   

Medcost

Many Medocst policies now cover Medical Nutrition Therapy at 100% (no co-pay, no coinsurance, no deductible) as a part of your preventative care.   We will verify if your 'employer group' has an agreement to allow coverage. 

Medicare

Limited to three hours the first year, and two hours each following year, with a diagnosis of Diabetes or Chronic Kidney Disease.  Requires a referral from your physician every year.

Humana

Folows Medicare coverage guidelines.  Limited to three hours the first year, and two hours each following year, with a diagnosis of Diabetes or Chronic Kidney Disease. Requires a referral from your physician every year.

Other Insurances 

Other Insurance Policies may be filed as a courtesy, but is not a guarantee of payment.

Contemporary Nutrition, Inc. will call your insurance company to verify coverage, but understand that this is a courtesy and not a guarantee of payment by your insurance company.  If these charges are filed and denied, the remaining balance will be billed directly to you.

REMEMBER:  If you "No Show" for your appointment or do not cancel 24 hours in advance, you will be charged a $50.00 "No Show" fee.  If you arrive with children under 12 years old, you will be rescheduled and charged a $50.00 "No Show" fee.  If you arrive more than 15 minutes late and do not have your paperwork completed, you will be charged a $50.00 "No Show" fee.  It is your responsibility to remember your appointment, we do not call and remind you.  If you do not pay any open balances within 90 days of billing, you will be sent to collections.  There is a $30.00 fee for each time an account is sent to collections.
